UPHS is a rigorous college-preparation program. Students interested in attending the school must be prepared to write an essay indicating their interest and willingness to participate in the school’s core learning activities, including high-level academics, leadership and community service.
Students and their parents or guardians will attend an entrance interview that will include UPHS and COS staff members.
To apply for admission to UPHS, you must complete the entire Enrollment Packet. Upon receipt of the completed package, an administrative review and interview will be scheduled.
All of these forms are pdf files. If you have Adobe Acrobat Reader, you may print the forms after completing them online. Alternatively, you may print the blank forms and complete them by hand. Please print legibly.
Please remember to sign and date the forms in the indicated places before submitting your packet for review.
